Spinal fusions have been performed for over 100 years. It is understood that in order for bone to form, motion needs to be minimized. Before screws and rods were developed casting and bracing were used to hold the spine stable enough to allow bone to grow between the separate bones and knit them into a solid whole. In the early days of scoliosis surgery, patients were kept at bedrest for 1 year in a cast. The development of effective spinal instrumentation and load sharing interdiscal fusion cages has allowed the surgeon to more reliably immobilize the spinal segments allowing time for the bones to grow together while allowing adjacent spinal segments to move normally. There are many different techniques to achieve a spinal fusion. They can all be used effectively and some spinal problems are more commonly fused in one way or another. Many times it is surgeon preference. I tend to choose what I know will give the best outcome with the least amount of risk.

These are some common examples of spinal fusions:

  • TLIF Transforaminal Lumbar  Interbody fusion
  • PLIF Posterior Lumbar interbody Fusion
  • ALIF  Anterior Lumbar Interbody Fusion
  • XLIF or DLIF Direct Lateral Interbody Fusion
  • OLIF Oblique Lumbar Interbody Fusion
  • PSF  Posterior inter-transverse fusion

Most of the above are combined with the PSF. The ALIF can be performed alone if stable fixation is added to limit motion.

The 360 fusion = ALIF + PSF + screws and rods. This represents the most stable reconstruction and leads to reliable fusion and outcomes as well as return to full activities without restrictions of any kind in most patients within 3 months of surgery.
